Appointment Information

Your Office Visit

Our entire staff is prepared to make your visits to our clinic and any subsequent surgery go as smoothly as possible. On your first visit, be sure to bring your insurance card and any information given to you by your referring physician. (Many insurance plans, for example, require a referral form from your physician.) Even though most patients are referred by a physician, we do accept self referrals. Your first visit will involve an examination by one of our surgeons. We will then set up a surgery date for you if surgery is warranted. Except in emergencies, you will not go to the hospital from our clinic.

Clinic Hours

Our clinic is open 8 a.m. to 5 p.m. weekdays. Except in an emergency, we prefer to see patients by appointment. This allows us to schedule you an appropriate amount of time with one of our physicians. If you cannot keep your appointment, please give us as much notice as possible.
